Student Requirements

All full-time students must submit a completed Health History form (.pdf) to facilitate medical treatment when away from home and to have easy access to their immunization history (i.e. required academic classes and athletic programs). All health information is held in the strictest confidence and will not be released without student's written consent (or court ordered subpoena) and will not affect admission status.

The Pennsylvania College and University Vaccination Act (.pdf) requires all students residing in university-owned housing to provide proof of meningitis vaccination or have a waiver of exemption for medical, religious, or personal belief reasons. This information is collected by the Office of Health & Wellness Services.

Failure to submit the completed Health History form will result in a hold being placed on the student's university account which will prevent university transactions until the completed form is submitted.

Students who have not had one or all of the required immunizations due to medical, religious, or personal beliefs must provide a letter of explanation to the Office of Health & Wellness Services, and may be asked to leave the university during an outbreak of one of more of the diseases.

For more information on the diseases and the vaccines associated with them, please consult with the Health & Wellness Services staff or visit the CDC website external website.
